If the employee goes off work because of the injury:
Does investigation include taking photographs, collecting witness statements and extensive statements from the injured employee?	Yes	No
Do you contact the person immediately to check on their treatment and well-being?	Yes	No
Is an assessment made to determine if the work was done the way it should have been done?	Yes	No
Are Work Method Statements reviewed to ensure that they are adequate?	Yes	No
Do you have a rehabilitation policy?	Yes	No
Do you have a Company appointed Medical Officer?	Yes	No
Do you have a preferred Rehabilitation Providor?	Yes	No
Is the employee’s doctor contacted within the first week?	Yes	No
